Instructions: Board meetings are the first Tuesday of every month. Please have your proposal submitted by the Friday before the board meeting. No login required — anyone can submit.

Please include an itemized list or any other details to help the board vote.

Prefer a paper form? Upload a photo or PDF of your completed proposal. Images are converted to PDF. An attachment may be needed if your request includes itemized costs or receipts.
